The Snoqualmie Valley Human Services Coalition is a growing group of human service providers, faith communities, medical and behavioral health providers working together in new ways to connect community members to the human services and other resources they need.
Navigation Program
The Coalition’s primary area of focus is the Snoqualmie Valley Referral and Navigation Program, which provides centralized access to human services in the Snoqualmie Valley. Community members may call our shared phone line or request that a partner organization submit a referral to the program on their behalf. The referral is directed to Case Managers at A Supportive Community For All who serve as “navigators,” connecting with the community member to gather information about needs and then assign referrals to partner organizations that are best suited to meet those needs. The navigators track referrals to completion and provide advocacy and case management support as needed. We are currently using a shared online platform, Julota, that enables partners to better coordinate with each other in service to community.

Collaborating across multiple organizations requires a commitment to building shared values and a culture that practices equity leadership skills, embraces learning, fosters resilience, and embodies community-led governance. We spend a significant amount of time attending to these areas because we believe they are critical to effectively meeting the needs of our community.
We believe that working together to be more coordinated and improve access to the key resources our organizations’ provide, widely understood to be social determinants of health, will contribute to the greater health and well-being of our community over time. Further, the centralization of referrals will allow us to aggregate and analyze data across the region on demographics, the efficacy of the referral, how long it took to fulfill, and other data points that can paint a fuller picture of resource provision in our region – and point to where there are gaps and opportunities.
The Coalition is led by A Supportive Community For All, a 501(c)3 organization that was formed in 2021 to provide backbone support to organizations working together in innovative and transformative ways to make human services more accessible. As the lead organization, A Supportive Community For All’s role is to guide partners towards a shared vision, provide training and coordination, serve as fiscal sponsor, and build capacity for our partners to practice equity leadership skills in their collaborative and individual work.
